The Debt and Mental Health Evidence Form (DMHEF)

A free form completed by a health professional that helps creditors treat you fairly.

Debts & Mental Health3 min readReviewed July 2026

The DMHEF gives creditors clear information about how your mental health affects your ability to manage a debt, without you having to explain it repeatedly.

Most GPs, community mental health teams and hospital consultants can complete one. Some may charge a small fee, though many do it free of charge.

Creditors receiving a DMHEF are expected to consider forbearance — pausing interest, reducing payments or writing off amounts in serious cases.
